The Olaf Boedtker Award for Excellence in Undergraduate Advising Voting Ballot

Carefully follow the instructions listed on the left side of the ballot below. You are being asked to rate your advisor by responding to a number of statements on a rating scale of 1 (strongly disagree) to 5 (strongly agree).

Important Info

A list of advisors in the College of Science can be found HERE. You may give an evaluation for someone not on the list if they have been your academic advisor.

Voting begins May 13th and ends June 3 . Please submit your ballot as early as you can.

If you have any questions about this ballot, or if you encounter any technical problems, please contact the College of Science Dean's Office at 737-4811 Monday through Friday 8am - 5pm.

You may vote only once.
Although this is an anonymous ballot, we are monitoring the voting system.
Any votes associated with suspicious activity will not be counted.
